等速肌力测试评价断裂跟腱修复后的肌肉力量

摘要：

背景：恢复跟腱的完整性和坚韧性以及踝关节的灵活性对跟腱断裂的结局尤为重要。

目的：实验采用等速肌力测试系统测试跟腱断裂修复后的肌肉力量，评价其修复效果。

方法：采用Biodex System 4多关节等速肌力测试系统，以60 (°)/s，120 (°)/s两种速度测定17例跟腱断裂修复后三四个月患者的屈踝和伸踝肌力，指标包括峰力矩、峰力矩/体质量比值、总功、平均峰力矩和单次最大功。

结果与结论：断裂跟腱修复后患者患侧屈肌群的峰力矩、峰力矩/体质量比、总功、平均峰力矩和单次最大功5项检测指标在60 (°)/s和120 (°)/s时较健侧均显著降低(P < 0.05)。结果证实，等速肌力测试可有效反映跟腱断裂修复后的肌肉力量情况，能评价跟腱断裂修复后的效果。

Abstract:

BACKGROUND: Restoration of tendon integrity and tenacity, and maintenance of ankle joint flexibility, play a crucial role on the rupture of Achilles tendon.

OBJECTIVE: To evaluate the muscular force using isokinetic muscle strength test after the ruptured Achilles tendon is repaired, and to assess the repairing effect.

METHODS: Muscle power including peak torque, peak torque/body weight, total work, average peak torque, set total work of flexor and extensor of 17 patients with Achilles tendon rupture were assessed after reconstruction using Biodex system 4 at 60 (°)/s and 120(°)/s.

RESULTS AND CONCLUSION: The peak torque, peak torque/body weight, total work, average peak torque, set total work of flexor muscle group at the injured side were significantly decreased compared with uninjured side at 60 (°)/s and 120 (°)/s (P < 0.05). Isokinetic muscle strength test can reflect the muscular force after the ruptured Achilles tendon is repaired, and evaluate the repairing effect.

引言

材料方法

设计：自身对照，调查分析。

时间及地点：于2009年1月至2010年4月在四川省骨科医院康复科完成。

对象：纳入17例跟腱修复后患者行等速肌力测评，其中男15例，女1例；年龄20-27岁，身高165-182 cm，体质量60-95 kg。包括左跟腱损伤患者8例，右跟腱损伤患者 8例。平均年龄23.4岁。

纳入标准：①均在四川省骨科医院行跟腱断裂修复的患者。②修复后三四个月的患者。

所有患者均在四川省骨科医院行跟腱断裂修复术。修复后根据患者情况指导行足跖趾关节的跖屈和背伸活动，股四头肌的收缩和放松练习，踝关节被动屈伸和内外翻活动度，肌力训练等。

材料：测试仪器为美国Biodex Medical Systems公司生产的Biodexsystem4多关节等速测试系统，测试前对系统进行常规校正。

方法：

等速肌力测定：患者17例测试前进行5 min的热身活动(骑功率自行车)。测试时患者坐在测试椅上，座位角度为90°，踝关节的轴心与动力臂的轴心一致，动力臂末端的阻力垫固定在足背。测试前对患者下肢进行肢体称质量，测试结果经下肢重量校正，以排除肢体运动时重力的作用。根据患者的活动度设置关节活动范围，测试速度为60， 120 (°)/s两种，每种速度为一单元。每次测试作5组最大收缩运动，两单元之间的间隔为60 s。在每种测试速度正式测试前，先进行3次亚极量屈伸练习作为热身活动，然后进行正式测试，先测健侧腿，间隔2 min后，再测患侧腿。检测患者在测试速度为60，120 (°)/s时的峰力矩(peak torque，PT)、峰力矩与体质量比(peak torque/body weight，PTBW)、总功(total work，TW)、平均峰力矩(average peak torque，AVGPT)、单次最大功(set total work，STW)。

主要观察指标：患者测试速度为60，120 (°)/s时的峰力矩(PT)、峰力矩与体质量比(PTBW)、总功(TW)、平均峰力矩(AVGPT)、单次最大功(STW)的变化。

统计学分析：计量资料用x(_)±s表示，运用STAT 6.0统计学软件进行数据分析，组间均数差异的比较采用配对t 检验，显著性检验水准α=0.05。

随着全民健身，越来越注重健康，人们生活中跟腱损伤的日益增多，其修复治疗和康复手段得到了很大发展, 跟腱断裂修复后的再生和重塑期合理安排训练十分重要，因为它可使断端应力增加，排列紊乱的胶原纤维按应力重新排列，有利于新合成的结缔组织选择性再塑型，早期恢复强度^[2]。因此，修复后进行循序渐进康复训练促进跟腱愈合和早日康复，而康复计划的制定依赖于一些客观主观指标评价。

肌肉力量的评价是检验康复效果的一个重要内容。对跟腱修复后的等速肌力评价，在国内较多应用于膝关节肌力评价及训练，在踝部损伤应用少见，等速肌力测试可精确定量测定关节周围肌力，对评价各种运动系统伤病及康复具有重要作用^[3-45]。等速仪器所提供的是一种顺应性阻力，阻力大小随肌肉收缩张力的大小而变化，产生的阻力根据使用者不同的运动力量而改变，故具有较高的安全性^[46]。

实验对16例跟腱断裂患者进行等速肌力测试发现,患者跟腱断裂修复后患侧屈肌群的峰力矩、峰力矩/体质量比、总功、平均峰力矩和单次最大功5项检测指标在60，120 (°)/s时均较健侧显著降低，发生这种肌力改变的原因主要由于跟腱损伤后的废用性肌萎缩。根据患者测得肌力指标对应的指导患者增加肌力、平衡、由慢走过渡到快走等训练。恢复跟腱断裂患者的肌力,对其恢复正常的运动能力和预防再损伤具有重要意义。实验结果显示，应用等速肌力测试可以对跟腱断裂修复后肌力康复效果作出有效评估，并依据其调整患者的康复治疗方案。
